2nd PCI express Intel NIC not showing in ESX

Hi,

I've installed ESX 3.5 on a HP DL380 G5 server.  The server has 1 x 2 port onboard broadcom NIC but I've also installed 2 x HP NC360T 2 port NIC adapters.  The 2 x broadcom nics show up fine but only one of the PCI express intel nics show up in ESX.  I see all 3 NICS fine in the bios and during POST.  I've tried different combinations of IRQ but I just can't get this 3rd NIC card to show up in ESX.
