KS.cfg file will not start

I have created a KS.cfg file from the "Scripted Install Creator".  When trying to install ESX on a server I type "ESX ksdevice=eth0 ks=http://<servername>/VMKS/KS.cfg", before the test media prompt I get "Sending request for IP information for eth0" (message I dont get without the aforementioned command).  Then the setup continues but I get prompted for the usual setup questions (ie, Mouse, keyboard, language settings, EULA) things I have specified in the KS.cfg file.  It appears as if the setup is not rading the config file.

 

 

 

 

 

Finally for my question, how do I know if setup is using my config file, and/or how do I get it to read the config file?  The file was converted using "dos2unix", I have also tried using a USB floppy drive.  I am installing ESX 3.0.2 on a server that previously had it (demoted for testing purposes) it was installed manually.  Please help, I very frustrated  .
