Ran into this issue at a customer on a set of IBM x3950 servers with 4Gibt Qlogic HBAs when installing VI3.  This was tricky to find, so I hope this helps anyone in a similar situation...

The following will occur when a 266Mhz adapter is installed in a PCIX slot and it is configured to run in Mode-2 or Automatic (Advanced PCIX Settings) during a Reset (Cnt/Alt/Del, Reset Button, Operating System Restart, Hot Plug Latch):

the server will poweroff

the PCIX LED will be lit on the Light Path Panel

a 1.5v Power Good Fault will be in the RSA and/or

Baseboard Management Controller (BMC) logs

RSA logs contain the following events:

1 INFO SERVPROC 04/09/07 10:08:55 Node powered off

2 INFO SERVPROC 04/09/07 10:08:55 System Complex Powered Down

3 ERR SERVPROC 04/09/07 10:08:49 1.5V Power Good Fault

4 INFO SERVPROC 04/09/07 10:08:49 Node reset

BMC logs contain the following events:

Solution

Update to BMC-05I or later for the following machine types:

xSeries 366/System x3850 (type 8863)

xSeries 260/System x3800 (type 8865)

xSeries 460/System x3950 (type 8872)

xSeries 460MXE/System x3950E (type 8874)

Update to BMC-20A or later for the following machine types:

System x3850 (type 8864)

System x3800 (type 8866)

System x3950 (type 8878)

System x3950E (type 8879)

The Target Date is scheduled for Second Quarter 2007.

Workaround

Run all 266Mhz cards in Mode-1:

Enter BIOS Setup

Select Advanced Setup

Select Advanced PCIX settings

Select PCIX Mode-2 Speed Capability Menu. There may be two different options:

a) Change Automatic Selection or Select Mode-2 to Select Mode-1.

b) Slotx Disable PCIX 266MHz Mode-2 from Disable to Enable.

Additional Information

Due to a sensitive controller on the 1.5v voltage regulator, the Power Good line is reacting to the Reset condition resulting in the dropping of the Power Good line and the server.

All 266Mhz Mode-2 adapters run off the 1.5v power rail. All 266Mhz Mode-1 adapters use the 3.3v power rail. Assigning Mode-1 eliminates the 1.5v glitch. Running the adapters in Mode-1 can result in a small performance impact.

The BMC fix eliminates the 1.5v sense. This error could be removed from BMC checking because it was a redundant power check. There is additional logic that handles all other power conditions (OC, OV) that may occur.
